Paper Reporting Methods

We will accept paper reports if there are ten (10) accounts or less valued at $100.00 or more.

You can complete the Report Coversheet (Form COT/ST 918 claim form (PDF)) and list the individual items/owners on the Detail Sheet (Form COT/ST 919 claim form (PDF)), and mail to the address below along with payment, or proof of payment.

An authorized officer must sign off on the report.
